Tenecteplase (TNK-tPA)

Description:
Tenecteplase is a group of drug called TPA (tissue plasminogen activators). These drugs work by causing the body to produce chemicals called plasmin in excess, to break down unwanted blood clots.

Indications:
To prevent deaths from heart attack (acute myocardial infarction).

Dosage:
Give as a single bolus through a vein (intra venous) for 5-10 seconds, as follows:

<60 kg: 30 mg (6000 u)
60 to <70 kg: 35 mg (7000 u)
70 to <80 kg: 40 mg (8000 u)
80 to <90 kg: 45 mg (9000 u)
≥ 90 kg: 50 mg (10,000 u)
Maximum dose: 50 mg (10,000 u)

Provide additional UFH in all patients:
60 u / kg by IV bolus (maximum dose: 4000 u)
Followed by: 12 u / kg / hour infusion intravenously (maximum dose: 1000 u / h), multiplied by 24-48 hours
Set to maintain a target PTT: 50-70 sec

Side Effects:
Hematologic (bleeding, especially at the puncture site, severe internal bleeding, intracranial bleeding has occurred); Other effects (fever, chills with pain in the abdomen and back); Securities GI (N / V)
Rare: allergic reaction (poultice, skin redness, urticaria and anaphylactic-like symptoms are rare and blood diseases.
Infusion can be attributed to the emergence of hypotension (either directly or as a result of reperfusion), bradycardia and arrhythmias may occur due to reperfusion.
Destroy a blood clot emboli occasionally cause anywhere, therefore requiring heparin prophylaxis.

Special Instructions:
Avoid in patients taking anticoagulant drugs ..
Contraindicated in patients with peptic ulcer, pregnancy, who had surgery less than three weeks, who suffered internal bleeding (eg GI) within 4 weeks.
