Heroin Treatment Works To Prevent Opioid Addiction

Diacetylmorphine injection, the active ingredient in heroin, which has been shown to be an effective alternative to oral methadone in opioid addiction overcome that did not respond to previous treatment.

Compared with methadone in a phase III trial, diacetylmorphine which leads to higher levels significantly reduce illegal activity and drug use after one year of treatment (67% versus 47.7%, P = 0.004), according to Martin Schechter, MD, PhD, from the University of British Columbia School of Population and Public Health in Vancouver, and colleagues.

Patients who received diacetylmorphine also had higher levels of retention in addiction treatment program in one year (87.8% versus 54.1%, P <0.001), the researchers report in the August 20 edition of the New England Journal of Medicine.

The values needed to treat of the two results is very low, according to Schechter - 5 for the reduction of illegal activities and drug use and 3 for retention.

According to Schechter methadone should remain the standard treatment for patients suffering from opioid dependence, and with diacetylmorphine reserved for 15% to 25% for those who do not respond.

"Methadone treatment remains a pretty good as the first treatment," he said, "but it's better to switch to heroin or using heroin in addition to increased effectiveness in this difficult population."

Joshua Lee, MD, from New York University, said of the validation of their research findings from Europe - that the treatment of heroin with a valid approach to treating addiction.

"In general," said Lee, who was not involved in the study, "it was a very positive development when a new treatment approaches that bring drug users into medicalized, and environmental therapy as further treatment may be established and associated problems such as the use of cocaine began to be overcome. "

Daniel Angres, MD, questioned the usefulness of both diacetylmorphine and methadone as a treatment option when an alternative with less potential for abuse - bupronorphine (marketed as Suboxone when combined with naloxone) and available.

Drugs are less likely to be abused, he said, because naloxone will prevent the effects of addiction bupronorphine if the medicine is injected.

To explore the treatment of diacetylmorphine in North American society, Schechter and his colleagues initiated the North American Opiate Medication Initiative (Naomi), is an open-label, in a phase III trial conducted in Montreal and Vancouver.

Experiments can not be done in the U.S. because of difficulties in obtaining financing and regulatory permission to inject heroin, says Schechter.

The participants of the long term as heroin users - meaning a period of 16.5 years who had at least two failed treatment efforts, one of which is owned methadone. About three-fourths had participated in illegal activities other than drug use in the months before the study began.

The investigators assigned to 111 participants on methadone and 115 on diacetylmorphine, which managed to three times a day with a maximum daily dose of 1,000 milligrams. And the treatment lasted one year.

At the end of treatment, a composite score for drug use and addiction disorders due to illegal activities in Europe with the Index increased significantly in both groups (P <0.01).

Patients who receive diacetylmorphine had been significant improvements in six of the seven sub-scales remained. Those who receive methadone improved in two things - the economic status and job satisfaction.

The advantage of diacetylmorphine groups larger than those in the methadone group for drug use, psychiatric status, job satisfaction, and social relationships (P ≤ 0.05 for all).

However, there is a more serious adverse events in groups of diacetylmorphine - 51 versus 18. None of the adverse events in the methadone group was due to the study treatment.

The most common serious adverse events with diacetylmorphine treatment of potentially life-threatening form of an overdose in 10 patients and seizures in six. But no patient died.

"We suggest that diacetylmorphine safety data should be delivered in settings where immediate medical intervention available," the researchers said.

Schechter added, however, that "we consider the results to show it is very safe."

Diacetylmorphine more expensive than methadone treatment, Schechter said. A year of treatment costs about $ 6,300 to $ 7,300 for the determination of heroin and approximately $ 2,700 to $ 3,600 for methadone.

But, he said, given the cost of untreated addiction to heroin is estimated around $ 50,000 per year - including the costs associated with medical care, prisons, and the legal process in which both, the treatment cost is very economical.

A formal analysis of cost effectiveness of sustainable, she said. In an accompanying editorial, Virginia Berridge, PhD, from the London School of Hygiene and Tropical Medicine, shows that European countries that have done similar experiments which have different responses in the outcome, and partly because of political and controversial nature of the treatment.

For example, she said, Switzerland and the Netherlands have begun to prescribe heroin as part of their medical system, while Germany and Spain have balked at the idea.

Heroin Addiction Therapy: Model Behavior

Addiction is a progressive, chronic relapsing disorders include symptoms mainly compulsions, uncontrollable and continuous use of alcohol and or other drugs despite adverse
(Substance Abuse Mental Health Services Administration).

INTRODUCTION
Between myth and reality, life in society has become addicted to the stigma that "Once the addiction is still addiction". They continued to use heroin will become addicted to behave (addict). In those who stop and start the recovery process must remember that when they use it again evoke disease of addiction. Therapy and recovery of heroin dependence is a very long process.

Rarely aware of the willingness of addicts who decided that one day will stop the habit of using heroin and will not use again. Most addicts should be forced to follow the therapy if it was related legal problems, the intervention of family members and friends, or tribulation force to change lives.

When addicts are confronted to control the use of heroin is the tendency to fail and uncontrolled then generally their response: "I know that I am in trouble, but I do not want to stop all this, I want to learn first how to use it only on holidays".

Here is a partial picture of the disease of addiction in the form of rejection reactions. Much like someone who was first diagnosed with malignant disease, "I might not have cancer." Someone heroin addict must go through various processes, often require several attempts before the therapy can overcome addiction and they can live without heroin.

SOME MODEL DEPENDENCE RECOVERY THERAPY
Model Lexington Kentucky (Public Health Service Narcotic Hospital)
In 1935 in the United States is a combination of hospital facilities, agriculture and prisons. Most patients were men who were imprisoned for Harrison Narcotics regulations. A small percentage of patients seek treatment themselves and are encouraged to follow therapy for 6 months.

At that experts describe as individual addicts who suffer from inferiority complex (inferiority complex) and using medications to reduce or cover up the feelings experienced interference. Furthermore, therapies designed 3 phase covering the first phase of detoxification, the second phase provides the skills, discipline and mental and physical build, the last phase of preparing the addict back to life in society.
Research in 1965 (John O'Donnell) gets around 80% of addicts relapse.

Synanon and the therapeutic community model
Beginning of the 50's a drug addict group agreed together to form Synanon, the one that started the therapy community in Visalia California. Group is led by Charles Dederich a narcotics addict who believes that the addict is better treated without the help of medication and psychiatric therapy.

Synanon referring to the Alcoholics Anonymous / AA and to design specific therapy when faced with manipulative behavior of heroin addicts (aggressive therapy group). Group relying only to counselors and leaders who are former addicts among them. This method is the forerunner model of therapeutic community in response to the health care system is not able to help drug addicts heavy (hard core heroin addict). This therapy model to be used as a tactic known as a superior to deal with the behavior of addicts. Synanon was developed in the 60s and was declared as a treatment of heroin dependence.

Twelve steps
As Synanon, other narcotic dependency group in Southern California found "Narcotic Anonymous / NA" in 1953 which have a common principle that is healing itself. Which contains 12 measures to overcome dependence. The main difference with AA is that addiction is viewed as a disease (disease of addiction) so it does not emphasize on the specific substance abuse. Any change in mood that trigger the use of substances should be prevented

12 steps of healing:

  1. We recognize that we are powerless over alcohol and our lives become disorganized.

  2. Believing in the power greater than ourselves could restore sanity.

  3. Making the decision to turn our will and lives to the protection of God as we understand it.

  4. Make searching and fearless moral attitudes into our own.

  5. Admitted to God, themselves and others how wrong we are.

  6. Completely prepares the Lord will restore damaged characters.

  7. Humble heart to ask Him to remove all our shortcomings.

  8. Make a list of everyone who ever hurt us and begin to fix it.

  9. Correcting / apologize whenever possible except when going to hurt them or others.

  10. Introspection on an ongoing basis and when we do wrong promptly admitted it.

  11. Seek through prayer and meditation to improve our belief in God as we understand it, pray only to find out what He wants for us and ask for strength to carry it out.

  12. Having a spiritual consciousness as a result of these steps. We tried to carry this message to alcoholics and implement in all the problems we face.

Now this drug treatment, sex addiction / eating / gambling and many other therapies which refers to the 12-step principles. 12 steps there is a way for people with dependency. So no doubt the 12-step principles of AA and have great impact on the recovery process of millions of people around the world.

Hazelden model and Minessota
Hazelden recovery focused psychological, spiritual, medical, and social sides of addiction. Teaching core addicts is to rely on the AA handbook for inspiration and guidance and demonstrate how a spiritual life are delivered directly through the 12 steps. At the addict to leave the rehab center Hazelden recommend half way house where addicts can live anywhere within a few months to years. Hazelden asserts that the key to success relies on to stop using drugs, to build family relationships with addicts in recovery, giving enough time to myself to stay away from drugs and changing living habits, and follow the instructions of people in AA / NA.

Alzheimer's drugs are Useful in the treatment of Heroin addiction

National University Hospital of Cheng Kung in Taiwan and the United States national institutes of health jointly develop a new treatment for heroin addiction based on a drug that is used to slow the progression of Alzheimer's disease.

Both these institutions have discovered, when every day consume less than five milligrams of Alzheimer's drugs, Ebixa (memantine), will be able to help heroin addicts use to reduce dependence on transitional therapy methadon methadon.

The drug is Methadon substitution of heroin and used to reduce the craving in heroin and any other actual opiates.

In clinical testing for three months by the hospital to the patient who is addicted to heroin, 90 found that 48 of them using the show a decreased reliance on Ebixa methadon, while 42 other patients who consumed a placebo became increasingly dependent on methadon.

In addition, Ebixa seem effective in protecting and regenerate nerves and noted that the researchers also noted the existence of a reduction in the number of neurotoxin (nerve poison) and such as perforin (cell poisons) in patients using the drug.

Transitional therapy, which started to use methadon in Taiwan since 2004, limited to eliminate drug addiction due to replace the sense of actual opiates anesthetic for a moment with actual opiates lasting, like methadon.

The hospital has taken a patent for the invention of memantine at play "protecting nerve degeneration caused by inflammation of the dopamine" in the United States and the European Union.

Dopamine nerve can be damaged by the use of the methadon, causing dependence on a substance, and Lu said that the discovery could help more people cured of addiction in the future if the new treatment was successfully promoted.
